Salt Lake County Birth Certificate Offices

Salt Lake County has the most vital records offices of any county in Utah for birth certificate access.

Three branch offices serve different parts of Salt Lake County. The Ellis R. Shipp Center is in West Valley City at (385) 468-3712. The South Redwood Center sits in West Jordan at (385) 468-5312. Sandy hosts the Southeast Center at (385) 468-4329. Each Salt Lake County branch handles the same birth record and birth certificate requests as the main office.

How to Order Birth Records in Salt Lake County

Salt Lake County Birth Certificate Requirements

ID is required for all Salt Lake County birth certificate requests. A government-issued photo ID is the primary form accepted. This includes a driver's license, passport, or military ID. Tribal ID cards also work. Without a photo ID, two secondary forms may suffice. A Social Security card or voter registration card can serve as backup for Salt Lake County vital records requests.

Under Utah Code Title 26, Chapter 2, Section 5, every live birth must have a birth certificate filed within 10 days. Only eligible people can request a Salt Lake County birth record less than 100 years old. Those eligible include:

  • The person named on the birth certificate
  • A parent listed on the birth record
  • A sibling, spouse, child, or grandparent
  • A legal guardian with proper documentation

The FamilySearch collection of Utah birth certificates from 1903 to 1914 includes Salt Lake County births. Searchable indexes and images are available. Correcting Salt Lake County Birth Certificates

Errors on a Salt Lake County birth certificate can be fixed. Use the state amendment process. You need an Amendment Application from the Office of Vital Records. Most amendments require a notarized affidavit signed by two witnesses. Fees apply. Submit the form to the state vital records office in Salt Lake City.

Amending a Salt Lake County birth certificate costs $27. That includes one corrected birth certificate copy. Major changes like a name change on a Salt Lake County birth record require a court order. File a petition and attend a hearing. Gender marker changes on a Salt Lake County birth certificate follow a separate process. An extra $40 applies to seal the old marker.

Salt Lake County Birth Records Fees

Salt Lake County charges $20 for a certified birth certificate. Each additional birth certificate copy is $10. This is lower than the $22 fee at most other Utah counties.
